Note: We do NOT see clients at this location. Instead, the Concord Hospice care team will come to your home or wherever your loved one needing hospice care currently resides. In other words, we provide needs assessments and hospice care at your location.

Our business office hours are Monday – Friday, 8 am – 5 pm.

At Concord Hospice, we accept Medicare, Medicaid, and most major insurance plans. Please contact your insurance company or you may call our office at (469) 906-2000 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express.
